• Účasť na podujatí

Prehlasujem, že nemám zdravotné problémy a že som dostatočne pripravený štartovať na tomto bežeckom podujatí, ktorého sa zúčastňujem dobrovoľne na vlastnú zodpovednosť. Oboznámil som sa s propozíciami podujatia a na podujatí sa budem riadiť a rešpektovať organizačný poriadok a pokyny organizátorov. Som si plne vedomý toho, že nesiem všetku zodpovednosť a rizika spojené s mojou účasťou na tomto podujatí a za prípadnú škodu spôsobenú na zdraví alebo majetku, ktorá vznikne mne alebo ju spôsobím organizátorovi, partnerom podujatia či všetkým tretím osobám, ktoré sa podieľajú na podujatí. Prehlasujem, že v prípade zranenia alebo poškodenia svojho zdravia alebo majetku si túto škodu nebudem uplatňovať u organizátora a tretích osôb. 

• Spracovanie osobných údajov

Ako dotknutú osobu v zmysle Nariadenia Európskeho parlamentu a Rady (EÚ) 2016/679 z 27. apríla 2016 o ochrane fyzických osôb pri spracúvaní osobných údajov a o voľnom pohybe takýchto údajov, ktorým sa zrušuje smernica 95/46/ES (ďalej len ako „Nariadenie o GDPR“) a zákona č. 18/2018 Z.z. o ochrane osobných údajov a o zmene a doplnení niektorých zákonov dávam svoj súhlas Tennis-Club Wolfsthal , IČO: xxx, adresa na spracúvanie mojich osobných údajov na nižšie vymedzený účel. Pre účely tohto súhlasu sa pod pojmom „Podujatie“ rozumie Wolfsthalský beh. Vymedzený účel: prihlasovanie a registrácia účasti na podujatí, spracovanie štartovnej a výsledkovej listiny podujatia, vytvorenie bežeckej zložky/konta (meno a priezvisko, titul, ulica, PSČ, štát, národnosť, dátum narodenia, pohlavie, emailový kontakt, telefonický kontakt, poisťovňa, spracovanie merania dosiahnutého času na podujatí, vyhodnotenia dosiahnutého času ako športového výkonu na podujatí, zverejnenie dosiahnutého času na internetových stránkach a iných sociálnych sieťach. Taktiež dávam súhlas na spracovanie osobných údajov za účelom zaslania výsledkov ako aj informačných či organizačných pokynov a propozícií na e-mailovú adresu, respektíve telefónne číslo pred, počas a po podujatí. Súhlasím, že moje osobné údaje môžu byť poskytnuté všetkým tretím stranám, partnerom a sponzorom organizátora. Taktiež dávam plné právo organizátorom zdieľať moje fotky z podujatia na internetové stránky, sociálne siete pre všetky tretie strany, či partnerom a sponzorom organizátora.

Súhlas vydávam dobrovoľne podľa zákona č. 136/2014 Z.z. o ochrane osobných údajov v znení neskorších predpisov.

• Práva organizátorov

Organizátori si vyhradzujú právo na zmenu a doplnenie organizačného poriadku, pokynov, propozícií podujatia, diskvalifikáciu účastníka, či zrušenie preteku.

• Prehlásenie účastníka

Poskytnutím osobných údajov pri registrácii na podujatie, účastník vyhlasuje, že poskytnuté osobné údaje sú správne, pravdivé a aktuálne, v opačnom prípade zodpovedá za prípadnú škodu, ktorú uvedením nesprávnych, nepravdivých alebo neaktuálnych údajov môže Organizátorovi podujatia spôsobiť.

Teilnahme an der Veranstaltung

Ich erkläre, dass ich keine gesundheitlichen Beschwerden habe und ausreichend darauf vorbereitet bin, an dieser Laufveranstaltung teilzunehmen. Meine Teilnahme erfolgt freiwillig und auf eigene Verantwortung.

Ich habe mich mit den Teilnahmebedingungen der Veranstaltung vertraut gemacht und werde während der Veranstaltung die Organisationsordnung sowie die Anweisungen der Organisatoren befolgen und respektieren.

Mir ist vollständig bewusst, dass ich sämtliche Verantwortung und alle Risiken trage, die mit meiner Teilnahme an dieser Veranstaltung verbunden sind. Dies gilt auch für etwaige Personen- oder Sachschäden, die mir entstehen oder die ich dem Veranstalter, den Partnern der Veranstaltung oder sonstigen Dritten zufüge, die an der Durchführung der Veranstaltung beteiligt sind.

Ich erkläre, dass ich im Falle einer Verletzung, einer gesundheitlichen Beeinträchtigung oder einer Beschädigung meines Eigentums keine diesbezüglichen Ansprüche gegenüber dem Veranstalter oder Dritten geltend machen werde.

Verarbeitung personenbezogener Daten

Als betroffene Person im Sinne der Verordnung (EU) 2016/679 des Europäischen Parlaments und des Rates vom 27. April 2016 zum Schutz natürlicher Personen bei der Verarbeitung personenbezogener Daten, zum freien Datenverkehr und zur Aufhebung der Richtlinie 95/46/EG, nachfolgend „DSGVO“ genannt, sowie des Gesetzes Nr. 18/2018 Slg. über den Schutz personenbezogener Daten und über die Änderung und Ergänzung bestimmter Gesetze, erteile ich dem Tennis-Club Wolfsthal, Firmen-ID: xxx, Anschrift: xxx, meine Einwilligung zur Verarbeitung meiner personenbezogenen Daten für die nachstehend genannten Zwecke.

Für die Zwecke dieser Einwilligung bezeichnet der Begriff „Veranstaltung“ den Wolfsthaler Lauf.

Festgelegte Zwecke der Verarbeitung:

Anmeldung und Registrierung zur Teilnahme an der Veranstaltung, Erstellung und Verarbeitung der Start- und Ergebnislisten der Veranstaltung, Erstellung eines Läuferprofils beziehungsweise Benutzerkontos sowie Verarbeitung folgender personenbezogener Daten:

Vor- und Nachname, Titel, Straße, Postleitzahl, Staat, Staatsangehörigkeit, Geburtsdatum, Geschlecht, E-Mail-Adresse, Telefonnummer und Krankenversicherung.

Die Verarbeitung umfasst außerdem die Zeitmessung bei der Veranstaltung, die Auswertung der erreichten Zeit als sportliche Leistung sowie die Veröffentlichung der erreichten Zeit auf Internetseiten und in sozialen Netzwerken.

Ich erteile ebenfalls meine Einwilligung zur Verarbeitung meiner personenbezogenen Daten zum Zweck der Zusendung von Ergebnissen sowie von Informations- und Organisationshinweisen und Teilnahmebedingungen an meine E-Mail-Adresse beziehungsweise Telefonnummer vor, während und nach der Veranstaltung.

Ich erkläre mich damit einverstanden, dass meine personenbezogenen Daten sämtlichen Dritten, Partnern und Sponsoren des Veranstalters zur Verfügung gestellt werden können.

Darüber hinaus räume ich den Organisatoren das uneingeschränkte Recht ein, Fotos von mir, die während der Veranstaltung aufgenommen wurden, auf Internetseiten und in sozialen Netzwerken zu veröffentlichen und Dritten sowie Partnern und Sponsoren des Veranstalters zur Verfügung zu stellen.

Diese Einwilligung erteile ich freiwillig gemäß dem Gesetz Nr. 136/2014 Slg. über den Schutz personenbezogener Daten in der jeweils geltenden Fassung.

Rechte der Organisatoren

Die Organisatoren behalten sich das Recht vor, die Organisationsordnung, die Anweisungen und die Teilnahmebedingungen der Veranstaltung zu ändern oder zu ergänzen, einen Teilnehmer zu disqualifizieren oder den Lauf abzusagen.

Erklärung des Teilnehmers

Mit der Angabe seiner personenbezogenen Daten bei der Anmeldung zur Veranstaltung erklärt der Teilnehmer, dass die angegebenen personenbezogenen Daten richtig, wahrheitsgemäß und aktuell sind.

Andernfalls haftet der Teilnehmer für etwaige Schäden, die dem Veranstalter der Veranstaltung durch die Angabe unrichtiger, unwahrer oder nicht aktueller Daten entstehen können.

Participation in the Event

I declare that I have no health problems and that I am sufficiently prepared to take part in this running event, in which I participate voluntarily and at my own risk.

I have familiarised myself with the event regulations and agree to follow and respect the organisational rules and the instructions of the organisers throughout the event.

I am fully aware that I bear all responsibility and risks associated with my participation in this event, including any personal injury or property damage suffered by me or caused by me to the organiser, the event partners, or any third parties involved in the event.

I declare that, in the event of injury, damage to my health, or damage to my property, I will not make any claim for compensation against the organiser or any third party.

Processing of Personal Data

As a data subject within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, repealing Directive 95/46/EC, hereinafter referred to as the “GDPR”, and Act No. 18/2018 Coll. on Personal Data Protection and on Amendments and Supplements to Certain Acts, I hereby give my consent to Tennis-Club Wolfsthal, Company ID No.: xxx, address: xxx, to process my personal data for the purposes specified below.

For the purposes of this consent, the term “Event” means the Wolfsthal Run.

The specified purposes are:

registration and enrolment for participation in the Event; preparation and processing of the Event’s starting and results lists; creation of a runner profile or account containing the following data: first name and surname, title, street address, postal code, country, nationality, date of birth, gender, email address, telephone number, and health insurance provider; processing of the participant’s measured finishing time at the Event; evaluation of the finishing time as a sporting performance; and publication of the finishing time on websites and social media platforms.

I also consent to the processing of my personal data for the purpose of sending results, information, organisational instructions, and event regulations to my email address or telephone number before, during, and after the Event.

I agree that my personal data may be provided to third parties, partners, and sponsors of the organiser.

I also grant the organisers the full right to publish and share photographs of me taken during the Event on websites and social media platforms, and to make such photographs available to third parties, partners, and sponsors of the organiser.

I give this consent voluntarily pursuant to Act No. 136/2014 Coll. on Personal Data Protection, as amended.

Rights of the Organisers

The organisers reserve the right to amend or supplement the organisational rules, instructions, and event regulations, to disqualify a participant, or to cancel the race.

Participant’s Declaration

By providing personal data during registration for the Event, the participant declares that the personal data provided is accurate, truthful, and up to date.

Otherwise, the participant shall be liable for any damage caused to the Event organiser as a result of providing incorrect, false, or outdated information.
